Ottawa Police member will be onsite to check I.D and forms.
If you are volunteering as Coach,Assit Coach,Trainer,On ice helper or Manager You will need one. Or if yours has expired. They are good for 3 years.
There will be another intake day in mid Oct, or once teams are made.
Required for Police Records Check.
1. Copy of volunteer form filled out online (typed out is better than hand written)
2 Photo copy of two pieces of Identification and bring these two pieces with you on the PRC intake night.
3 RMMHA volunteer letter not required on these nights.
4 After you submit required paper work, Police Records Check Form will be mailed to you from Ottawa Police. It may take up to 10 weeks before it is returned. It will be up to Show RMMHA form and or follow up if you do not receive it back.

Important Notice: Ontario Health Cards cannot be accepted for identification purposes (Personal Health Information Protection Act, 2004 section 34(4)).
